1. Introduction

Erectile dysfunction (ED) and impotence (ED+impotence) are the two major disorders which have become the most frequently occurring and serious health problem for men in their thirties. ED has been attributed to the male reproductive hormones (HRT). Male sexual arousal, however, is also involved in ED and is caused by various factors, such as physical and psychological factors.

ED causes ED by the loss of an erection, which usually occurs between the beginning of sexual intercourse and continues to sustain the erection throughout the duration of sexual activity. For men who have difficulty getting an erection, ED is characterized by insufficient blood flow to the penis, which is a significant factor in the development of impotence and ED. For men with ED who have difficulties getting an erection, sexual stimulation, arousal, or sexual arousal, may result in the occurrence of ED.

In general, ED is defined as a reduced ability to achieve and maintain an erection during sexual activity, which occurs without the presence of sexual arousal. It is commonly found in men aged 18 to 64 years old, and it is more common in older men. ED+impotence occurs in two forms: one of them is characterized by an inability to attain and maintain an erection during sexual arousal, which usually begins with the loss of an erection in the beginning of sexual intercourse, followed by the loss of a normal erection within 30 to 60 minutes of sexual arousal. In contrast, the other form of ED is characterized by an inability to achieve and maintain an erection during sexual arousal, which generally starts during the early phase of sexual activity and can last up to 4 to 6 hours. Although ED is generally considered a relatively common sexual disorder that affects men of all ages, it is not considered a normal or inevitable part of aging.

In recent years, several studies have investigated the relationship between ED and male-pattern disorders, including the effects of erectile dysfunction medications such as phosphodiesterase type 5 (PDE5) inhibitors or antidepressants. In this study, we aimed to explore the effects of erectile dysfunction medications such as Viagra, Levitra, and Cialis on the occurrence of ED+impotence. The study included 160 men aged 18 to 64 years old, including 40 men who had both ED and impotence. The men had ED and ED+impotence based on a questionnaire conducted by the National Center for Health Research and Education, National Institutes of Health, the results of which were reported by the National Institute for Health and Care Excellence.

2. Results

The men in this study were selected based on the criteria outlined in the NICE guidelines for the prevention and management of male impotence. According to these criteria, ED+impotence was defined as a total of 4 to 6 hours of sexual activity and the duration of sexual activity lasting approximately 4 hours was determined. ED was defined as the occurrence of one or more of the following conditions:

  • Sexual arousal disorder
  • Hypoactive sexual desire disorder
  • Hypoactive sexual activity disorder
  • Lack of erection, or erectile dysfunction, associated with the occurrence of ED
  • Prolonged or painful erection (lasting 4 hours or more in 24 hours)

Among men who were diagnosed with ED and ED+impotence, there were 42 men in the study who met all these criteria. Only four of the men were in the group who were in the ED+impotence group (20.7%), with the remaining 31 men (18.9%) being in the ED+impotence group. In addition, in this study, the men in the ED+impotence group did not have any other types of ED, such as erectile dysfunction or ED+impotence. In addition, only one of the men had a history of heart disease, including a history of coronary artery disease and heart failure. The remaining men were excluded from the study due to the following reasons:

  • Men who were diagnosed with ED and ED+impotence had an age of 25 or more;
  • Men who had sex with men (MSM) in the past 3 years;
  • Men who had a history of alcohol abuse or a current smoking status;
  • Men who had a history of sexual partner abuse; and
  • Men who had a history of cardiovascular disease or a history of cardiovascular disease.

Among men with both ED and ED+impotence, the frequency of the condition was higher in the ED+impotence group than in the ED group (17.8 vs. 5.3%).
